2. CHECKLIST FOR THE SPONSOR
Your sponsor must complete and sign the following form:
( ) Form 40SP - Sponsorship for migration to Australia
Please send the following documents with the form:
( ) 1 recent passport-sized photograph
( ) evidence that your sponsor is an Australian citizen or permanent resident
(a certified copy of the birth certificate or citizenship certificate or passport),
or an eligible New Zealand citizen (a certified copy of the personal details
page of the passport)
( ) Statement on how your sponsor proposes to fulfil the sponsorship
obligations
Sponsorship Obligations
1. The sponsor undertakes to assist the applicant to the extent necessary
financially during the period of 2 years immediately following the
applicant's entry to Australia. This may include financial obligations to the
Commonwealth arising from the applicant's stay in Australia.
2. It is the sponsor's responsibility to ensure that the applicant complies with
Australian employment laws and the conditions imposed on the
applicant's visa.
Evidence to substantiate the sponsorship obligation statement may
include copies of: bank statements, investment portfolios, property
deeds, work contracts,evidence of professional qualifications, support
from family with appropriate evidence
Note: If your sponsor currently lives with you outside of Australia, he/she
should provide evidence of his/her current employment and information
on future employment in Australia (e. g. evidence of signed work contract
in Australia, contacts/arrangements made with future employers,
arrangements for initial accommodation). If the applicant has
arranged employment already, this information should also be included.
( ) Best interests of the child
Stronger Measures for the Protection of Children
Where an applicant includes a dependent family member under 18 years
of age in their application, there is requirement that there is no compelling
reason to believe that the grant of the visa would not be in the best
interests of that dependant. In order to assess this requirement, sponsors
of children under 18 years of age are required to submit a National Police
Check.

Is your sponsor (partner) an ‘eligible New Zealand citizen’?

Eligible New Zealand citizen sponsors must provide additional documentation:
( ) Form 80 - Personal particulars for character assessment
( ) original police clearance certificates for every country you have lived in
for at least 12 months (either consecutively or cumulatively) in the last 10
years

( ) Medical examination(s) with one of our panel doctors outside Australia or
HSA in Australia

You can choose to complete your medical assessment either before or
after lodging your partner’s application. Completing the assessment in
advance may speed up processing. However, the decision remains at your
discretion and cost, as the costs of medicals are not refunded if the
application is refused.
Note: Health checks (medical examination, x-rays and HIV tests) must be
conducted by an approved doctor. Depending on where you live, you may
need to make separate appointments to undergo a medical examination,
x-ray and HIV test.
Complete the following forms for each relevant person and take them to
the medical examination:
( ) Form 26 – Medical examination for an Australian visa
( ) Form 160 – Radiological report on chest x-ray of an applicant for an
Australian visa
