3. DEPENDENT CHILDREN AND OTHER DEPENDENTS
For each dependent applicant / child included in your application you must provide:
( ) 1 recent passport-sized photograph
( ) a certified copy of the birth certificate or adoption papers (if applicable)
( ) certified copies of the current passport or travel document
( ) for dependent children under the age of 18: evidence that you have legal custody AND have lawful written permission from any other person with custody rights (i.e. a statement signed in the presence of a notary public) for each child to migrate to Australia with the main applicant (unless you and your sponsor are the natural parents named on the birth certificate)
( ) if the dependent is aged 18 years or over:
Form 47A - Details of child or other dependent family member aged
18 years or over (to be completed by the dependant)
( ) if the child is aged 16 years or over:
Form 80 - Personal particulars for character assessment
(to be completed by the dependent child)
( ) if the dependent child / family member is aged 16 years or over:
an original overseas police clearance certificate for any country where the
person has lived in for at least 12 months (either consecutively or
cumulatively) in the last 10 years as a 16 year old or over
