 Police clearance certificates from every country where you have lived for more than 12 months in total (cumulative) during the last ten years.

 Some applicants are also required to complete a separate declaration regarding
character.

 One passport photograph from both sponsor and applicant.

 Statement on how your sponsor proposes to fulfil the sponsorship obligations: 

   Sponsorship Obligations

1. The sponsor undertakes to assist the applicant to the extent necessary financially during the period of 2 years immediately following the applicant's entry to Australia. This may include financial obligations to the Commonwealth arising from the applicant's stay in Australia.

2. It is the sponsor's responsibility to ensure that the applicant complies with Australian employment laws and the conditions imposed on the applicant's visa.

Evidence to substantiate the sposorship obligation statement may include copies of:

 The Relationship Pro-Forma, if you are in one of the categories mentioned
   on that form.

 Medical examination(s) with one of our panel doctors.

   You (and any dependent family members whether they are included in your
   application or not) should attend a medical and X-ray examination with one of
   our panel doctors. Children under the age of eleven are not required to undergo
   the X-ray examination. HIV testing is required for all applicants aged 15 and
   over. You will be responsible for paying the cost of the examination yourself -
   regardless of the outcome of your application.
