1. CHECKLIST FOR THE APPLICANT
You must complete and sign the following forms:
( ) Form 47SP - Application for migration to Australia by a partner
Important note for the applicant:
In Question 82. please write the full addresses and the dates of
residence for every place you have lived in the 10 years preceding the
date you lodge your application.
( ) Form 80 - Personal particulars for character assessment
( ) Form 886 – Settlement Details
Please send the following documents with your forms:
( ) 1 recent passport-sized photograph
( ) a certified copy of your current passport or travel document
( ) a certified copy of your birth certificate or an original extract from the
population register
( ) a certified copy of your name change certificate (example: deed poll) if
you have changed your name since birth
( ) original police clearance certificates for every country you have lived in
for at least 12 months (either consecutively or cumulatively) in the last 10
years (to be provided with the application)
Do you have your original police good conduct certificates from each country where you have spent 12 months or more (cumulative) in total in last 10 years?
For example: 5 visits to Australia of 3 months each in the last 10 years would mean your total stay was 15 months. Therefore, you would need to provide a police clearance from Australia. We encourage you to apply for any necessary police clearance certificates as soon as possible because it sometimes takes a long time to obtain them.

( ) Medical examination(s) with one of our panel doctors will be requested as the assessment of your application progresses.

( ) a certified copy of your military service discharge/completion certificate -
please provide such document if you have served in the armed forces of
any country in the past 10 years
Note: If you are a national of a country with compulsory military
service and have not served in the armed forces, please provide a
certified copy of your civil service duty or a certificate verifying exemption
from military service. This is also applicable for countries in which the
compulsory military service has now been abolished, but was in place
within the past 10 years.

a "Militäramtliche Bescheinigung" to:

You should also include a copy of your Dienstbüchlein and Swiss passport
for identification purposes and your postal address to receive the
confirmation by mail.
